Chronic illness is the term used to define many health issues such as Diabetes, Chronic Obstructive Pulmonary Disease, Heart Failure, Asthma, Chronic Kidney Disease and many more.

 

Unfortunately, when not controlled, these chronic issues lead to acute flare ups which we often see in the hospital setting.

 

What do we do at Medi-Call Direct?

We focus on keeping these issues well controlled and keeping you out of the hospital. But we don’t stop there!

If you do end up in the hospital, we work tirelessly to ensure your care is properly managed and all the right questions are being answered. We advocate for YOU, every step of the way!

 

Training and working in the healthcare system, we are used to advocating for our patients and their families.  It was when our own loved ones got sick that we were faced with a new perspective on a broken system.

The source of so many avoidable issues is lack of effective communication between a healthcare team, patients and their families.

Our goal is to help bridge the gap by taking the time to guide you in your health journey.

As a Board Certified Family Medicine physician, I carry a holistic perspective into the art and practice of Medicine.

The world we live in is continuously changing in many ways. There are many factors of life we cannot change or plan for, so let’s focus on the ones we can.  Taking our health into our hands starts with being empowered with the knowledge and understanding of our individual body and everything that affects it.  From genetics, social history, and environmental influences to medications, substances and disease processes, it all starts with gaining a clear understanding. The old saying holds true, “Knowledge is power”.

We collaborate with your primary care physician, specialists and even hospital admission team to make sure your goals are met every step of the way.

How long are your membership programs?

Each program is developed uniquely for each member and in collaboration with them. This is done in order to meet your specific needs at every phase of your life.

How do I join? What is the program fee?

Programs are highly customizable, the average program starts at $375.00 for 3 months! For every 10 memberships, one is provided free of cost to a household of 4 or less who cannot currently afford it. In order to join, we ask that you contact us for availability. The next step is filling out a short, confidential questionnaire.  The waitlist to join is typically 1-2 weeks.

Do you offer medical record review as part of the program?

We do include a thorough medical record review for our members. It is important to note that we do not offer medical advice or medical recommendations and ask that you discuss questions that arise during our review with your medical provider/physician/team.

What happens if I am admitted to the hospital?

We offer patient care advocacy both in the outpatient and inpatient setting for our members. We do not offer direct or indirect medical care, nor do we provide medical recommendations or advice.  What we do offer is the initiation of direct communication with your hospital team to ask the questions we would ask if it were our family in the room.  We then communicate the information back to you and your loved ones so that everyone is on the same page.

*It is necessary to provide verbal permission for your medical team to discuss your care with us.
